INSTRUCTIONS

Enter date of activity, a brief description of the activity, the number of hours spent doing the activity and the points you should receive for the activity. You can find the points scale on the Chapter web page (www.pvacf.org) under the About Us tab. Click on Policies and Procedures and then Sports. Read the policy and procedure for reporting hours and the points system attributed. Once you have completed the form, scan it and email it to the Chapter office at office@pvacf.org. Or you can mail it to the office at 2711 S. Design Ct. Sanford, FL 32773.
